Advance care planning is a process that supports individuals to receive medical care that is consistent with their values, goals, and preferences. It requires communication between the individuals and their loved ones. As well as their healthcare team.
Holding space for the dying
I will walk alongside you and your loved ones during your journey. I will help create a sacred space, find local community resources, plan vigils and other end of life services.
